Scaling or GBT hygiene: what is the difference for your teeth?

9 October 2026

Conventional scaling removes tartar, the hard deposits that build up on teeth. The GBT method (Guided Biofilm Therapy) goes further: it first makes the biofilm visible, the invisible plaque behind most cavities and gum problems, removes it with Air-Flow technology, and then takes off any tartar that is left. At Clinique Keller, our dental hygienist uses the GBT method.

The difference at a glance

– Conventional scaling GBT method
What it targets Tartar that has already formed The biofilm first, then any remaining tartar
Biofilm made visible No Yes, with a disclosing agent
Technique Instruments, often ultrasonic Air-Flow (air, warm water, fine powder), then ultrasound only where tartar remains
Between teeth, gum line, implants, braces Harder to reach Well reached by the Air-Flow jet
Stains (coffee, tea, wine, tobacco) Depends on the final polish Removed along with the biofilm
Brushing advice General Tailored, based on the stained areas
Comfort Can bother sensitive teeth Usually better tolerated

The two are not opposites: the GBT method includes scaling, but only where tartar remains, once the biofilm has been removed.

Why biofilm matters more than tartar

Biofilm is a layer of bacteria that settles on teeth, gums, the tongue, implants and braces. It builds up again every day, even in people who brush well. If it is not removed, it can cause cavities, gum inflammation (gingivitis, often with bleeding) and later disease of the tissues that support the teeth. Tartar is hardened biofilm: dealing with the biofilm means tackling the problem at its source.

What happens during a GBT session

  1. Disclosing agent: a harmless dye makes the biofilm visible, for the hygienist and for you.
  2. Tailored advice: using the stained areas, the hygienist shows you where your toothbrush or floss is missing.
  3. Air-Flow: a jet of air, warm water and fine powder removes biofilm and stains, including between the teeth and along the gum line.
  4. Targeted scaling: any hardened tartar left is removed with ultrasound, only in those places.
  5. Check and rinse: your mouth is checked and rinsed at the end of the session.

A scientific review of the GBT method (2021) describes these steps and reports outcomes that are comparable to or better than conventional scaling and root planing, with less pain reported by patients.

Who is the GBT method for?

  • Adults who want thorough preventive care.
  • People with implants, bridges or dentures.
  • Patients followed in periodontology (gum treatment).
  • People wearing fixed or removable braces.
  • People with stubborn stains or sensitive teeth.

The right treatment for your mouth is always decided after an examination by the dentist or the hygienist.
